IT TAKES A PLAN: Heartland Law Firm and the Midwest CareCenter are partnering with North Shore Village to provide a briefing on Estate Planning Essentials and POLST ((Physician’s Orders for Life-Sustaining Treatment). Matthew R. Margolis, Senior Elder Law Attorney at Heartland Law, will offer guidance on how to ensure that your estate is handled according to your wishes.
“Everyone thinks that a will is all they need,” Margolis said. “But it’s important to understand the difference between what a will actually does for you, as compared to a trust. Then you can make informed decisions about what is appropriate for you and your loved ones.”
Linda Rockwell, MPA, Community Events and Educational Outreach Manager with Midwest CareCenter, will explain the POLST form, a medical order that documents your wishes for your care in critical medical situations. The POLST form is an advance care directive in compliance with Illinois law.
“POLST becomes medical orders after you, your doctor and a witness sign it, and it obligates the medical system to follow these orders,” Rockwell explained. “Your choices should be based upon your knowledge of your medical condition, the likelihood that you will respond to medical treatments and your preferences. If your medical situation changes, or if your wishes and thinking change, POLST orders may also be changed.”
